change messy CREATEPDE macro to clean little function.

forget about the dirtypde bitmap and WIPEPDE/DONEPDE macros too.

check if mapping happens to already be in place, and if so, don't
reload cr3 (on the account of that mapping, that is).

don't reload cr3 unconditionally.
